BOV Leaking?

I just swapped over a Volant COI on my truck last night. Now under heavy load my TurboSmart BOV is getting blow-by at middle to high RPM's. What the heck? I can't see anything loose but I'm sure it's not the BOV it's been perfect for some time.

What could I have left loose, crazy?

Edit: I should have said it is venting to air under load.
