The lugs being loose or coming loose would have provided a warning because you would have felt the wheel moving around on the lugs before it actually came off, at least I would think so. Having all of the lugs shear off like that is due to over tightening them. Whoever was responsible for tightening the lugs the final time before they sheared is who is responsible for this issue.

With that wheel not mounting flat to the surface the stud is pulled through until 1. the wheel bends to make contact or 2. they tightening till the impact stops. Whom ever installed could have very easily kept tightening the lugs till they hit the correct torque all the while stressing the stud by pulling it through the hub...
Wheel looks incorrect to me...

Exactly right, if the wheels didn't fit properly it would start there, whether installed correctly or not.

Come on Gentlemen, pay attention, the pictures tell the story!
You can clearly see that the holes in the wheels are larger than the studs.
You can see the wear from the bearing hub spigot in the wheel hub (the black ring in the pictures)
You can clearly see the threats imprinted in one of the stud holes in the wheel.
I’m not talking loose lug nuts that you can feel as vibration; I’m talking under torqued lug nuts that allow the wheel to shift back and fore under acceleration and braking.

Come on ya'll, yeah it sucks, but this is obviouly a case of overtightened lugs or **** poor wheel design being lug centric without hub rings, or wrong seat lug nuts. I have seen this qute a few times and almost everytime it was because some idiot technician installed the lugs with an impact overtorquing them. Out on the road one lets go and the rest follow. More common than you think. Ford absolutely has a case to deny the claim. And even if the wheels were installed at a Ford dealer (who buys wheels from a dealer) it's the dealers fault, not a Ford warranty claim. Wake up here guys.

Happened to me in my Chevy Colorado, had to get new studs put in on a few hubs because some idiot thought he was at nascar and didnt hand thread the lugs and just put gun to stud. I tried to take my tire off when I got a flat (slow leak) and the lugs were stripped on. Drove 30 miles on 3 lugs, other 3 were loose and stripped. Had it towed home on a flat bed after getting out of Johnson Valley. Driving on 3 lugs has all sorts of vibrations that can cause the other lugs to snap or break the tow guy said. The tow truck driver also went in to 2 hours of stories of who died along the freeway, the entire way back to LA. The tow drivers stories made for a great cheery time for the girlfriend and I after my grandpa passed away on the same highway months earlier.

I agree, under torqued like hand tightened, and NOT torqued to spec.. Tight enough to hold wheel and tire from wobbling, but not tight enough from rocking the studs back and forth causing fatigue, and breaking..

And having incorrect lugnuts would have it to shift back and forth without being centered...

Gotcha! Thanks for the Clarification. i will speak with wheel manufacturer Monday and see
if they required spacers.
