Would those still fit with the stock spare under the bed or would you have to remove the spare?

Yes, but it would be 3-4” away, which I’m not sure would be a problem. But I took it off. If I go on a road trip I’ll toss it in the bed.

I just did this. I first saw it in person on Mkarban's truck. I was sick of ruining the tail pipes when going offroad.
It's a very simple mod, the exhaust doesn't even need to be cut. You just bolt these on near the rear axle before the spare tire.

I'll take some pics later but it's pretty straight forward. Exhaust sound is slightly more noticeable but i don't see any real negatives to this setup.
Did these slide into the tail pipe and you used the existing clamp? So the opening is 3"?
