Quick update in case someone finds this thread in the future with a similar question, I emailed Forged Offroad and they confirmed 2 brackets/spacers per side. One on top of the shock, the other on top of the frame.

The installation video posted by Forged Offroad also appears to show one of the brackets between the shock and the tower and a 2nd on top of the tower. Just seems odd they don't show 2 per side in the "official" parts diagram.

I have the factory resonator in place, but my truck came with an AFE CAI when I bought it, and that didn't help. I purchased an OEM intake that improved the sound, but still not ideal. I have been down shifting from 6th to 5th to keep the rpm's above the 2k mark as the best solution.
