clunk in rear end

I have found a clunk in rear of truck when i start it and back out of driveway. when i touch the brakes it has a loud clunk for the first few applications?
Secondly front end clunking when going over rough streets or speed bumps?
Any suggestions ?
Have read a few in the forums. I wish the dealers would look here.

its "the clunk".

Need the new driveshaft/slip yoke., I forget the TSB but its a well known issue. Google it for videos, play by play, colorful commentary, etc.
