O2 Sensor P2245 code

Getting the O2 Bank 2 sensor 1 error code at only 38K miles

A couple questions:

1. Can someone confirm that is the passenger side front O2 sensor?
2. Is it common to fail this early?
3. Is it common to indicate an issue upstream in the engine?
4. Easy fix? How?

It's more likely it's the wire.
Reference Voltage Circuit / Open (Bank 2 - Sensor 1)
The O2 is right next to the starter.
