Why is the center pin showing and not in the axle mount? This looks like a bad install that caused the issue. The leafs aren’t even centered in the mount. Wow!

There is not enough info. Yeah I see the loose washers. But was it left like that which I find hard to believe. Or was the picture taken when they were removing the leaf which he never said that they had a replacement.You need to look a little closer before you accuse anyone of a bad install.
The spring bolts are loose and the axle is dropped. You can see where the spring was touching the pad. There is no way the spring could be installed with the pin behind the perch because the axle would be too far forward.

Yes, you can see the metal fatigue surrounded by more rust then actual metal. I think the op has a case for new deavers, if he decides to contact them. OP: suggest if you put new deavers on, that you use some sort of rust protectant on them, like fluid film or equivalent. Those could of been a bad batch though.
