GEN 1 2010 5.4L heater core issues

As the title states, I've been having heater core issues. Long story short I had the heater core replaced under extended warranty mid last year. Then the core gave out and had it replaced under the part warranty a month ago (also both times the dual climate switch messed up giving one side warm air and the other cold air)
Now the heater core gave out yet again and will be replaced for the 3rd time in a year. Anyone saavy with heater cores that can help out to as why it's failing?

I'll take a look when I get it back from the dealer. I did read about a TSB. This TSB is "electrolysis"/ "Galvanic action".
